Title : PARATUBAL TORSION CYST WITH NORMAL OVARY IN THE FIRST TRIMESTER OF PREGNANCY: A CASE REPORT

Abstract :

Parametrium adnexal mass has a reported incidence of 0.05 to 3.2% during pregnancy. Paraovarian/paratubal cysts founded around 10% of parametrium adnexal masses and are often small and asymptomatic. Rarely occurring during pregnancy, a large paratubal cyst exacerbated by adnexal torsion is frequently misdiagnosed as an acute appendicitis or an ureteral colic4 that causes severe abdominal pain, hemorrhage or perforate. We report the case of a 29 years old female with torsion of paratubal cyst in the first trimester of pregnancy. The size of the mass 9.54 x 6.87 cm founded on ultrasonography that caused acute abdominal pain led to explorative laparotomy. Left partial salpingectomy and extirpation of the left intoto paratubal cyst was successfully done. The anatomic pathology examination revealed a serous cystadenoma with torsion of all structures. Most paratubal cysts are asymptomatic unless it has already enlarged significantly. Pregnancies with cysts larger than 10 cm and clinical suspicion of torsion require surgery. It presents a dilemma to the obstetrician. Surgery must be used judiciously and timed so that fetal risk of adverse outcomes are minimised.
