Title : A Review Pharmacological Activities of Melastoma malabathricum

Abstract :

The plant family Melastomataceae is exclusively found in tropical and subtropical regions. Melastoma is made up of 22 species, 2 subspecies, and 3 variations, all of which may be found in Southeast Asia. There was no evidence to substantiate the claim that this plant was one of the most essential herbs in traditional Malay, Indian, and Chinese medicine. Melastoma malabathricum has a variety of pharmacological properties, including antiviral, antioxidant, hepatoprotective, wound healing, antidiareeheal, antiinlamatory, anticancer, and antidiabetic activity. Melastoma malabathricum contains phytochemicals such as naringenin, quercetin, rutin, kaempferol, and ellagic acid. Despite the fact that various bioactive chemicals from Melastoma malabathricum have been isolated and identified (as detailed in the phytochemical section), their contribution to the plant's putative therapeutic advantages or recognized pharmacological effects has not been thoroughly explored.

Title : Relationship Between COX-2 Expression with ER, PR, HER-2, and KI-67 Expression in Breasr Cancer Patients at Dr Saiful Anwar Malang Hospital

Abstract :

Breast cancer is a malignancy of the breast which is one of the leading causes of death in women. COX-2 plays a role in increasing cell proliferation, delaying apoptosis, increasing angiogenesis, and increasing invasiveness of surrounding cells. Non-steroidal anti-inflammatory drug (NSAID) or anti-COX-2 therapy has the potential to reduce COX-2 overexpression and decrease breast cancer progression. COX-2 expression is thought to be related to hormone receptor expression of ER, PR HER-2, and Ki-67. The purpose of this study was to determine the relationship between COX-2 expression and ER, PR, HER-2, and KI-67 expression in breast cancer patients at DR Saiful Anwar Hospital, Malang so that it can be used as a basis for NSAID therapy. This study using cross-sectional study and carried out with a time span of July-November 2021 at the Anatomical Pathology Installation of Saiful Anwar Hospital, Malang. Sampling using consecutive sampling method with a total sample of 30 samples of breast cancer tissue. COX-2 expression with ER, PR, HER-2, and KI-67 was analyzed using Kendall's tau B with a significance level of p<0.05. There is a correlation between COX-2 expression with PR (r=-0.382; p=0.037) and Ki-67 (r=0.532;p=0.004). There was no correlation between COX-2 expression with ER and HER-2 receptors (p>0.05). COX-2 expression was negatively correlated with PR and positively correlated with Ki-67.

Title : Comparison between two-dimensional speckle tracking echocardiography and technetium sestemibi myocardial perfusion imaging in detecting coronary artery disease

Abstract :

Speckle tracking Echo is a present-day hand-held ultrasound imaging echo approach that permits a quantitative and objective assessment of local and global myocardial characteristics impartial from the angle of insonation and translational movements of the heart. It presently predicts coronary artery disease, and with the wonderful software program, software utility, this technique may be extra accurate for diagnosing and ruling out suspected coronary artery disorder (CAD). We aimed to compare 2D (two-dimensional) speckle-tracking echocardiography with a myocardial perfusion scan for the detection of suspected coronary artery stenosis. This study was carried out in National Heart Institute and Zagazig University hospital in Egypt between Jan 2021 and Jan 2022. We consecutively enrolled 120 patients underwent 2D speckle tracking echocardiography, technetium myocardial perfusion scan and coronary angiography study of suspected coronary artery stenosis patients and they were divided into two groups, group (1) is control one, and group (2) is diseased one. Two blinded operators generated a 17-segment bull's-eye map of longitudinal strain (LS), SPECT images, and coronary angiography films (reference). Comparisons between regional, global longitudinal strain and SPECT have been done. There was a statistically significant difference in global longitudinal strain (GLS), of a group (2) and proximal (basal, middle, and distal) LAD lesions (segment #17), and mean ± SD for the GLS in a group (2) turned into -14 ± 6.5, making the P-value surprisingly highly significant (P < 0.01), and a mean ± SD -10.4 ± 4 of proximal LCX lesions in segment #3, the p-value is enormously significant (P < 0.01). No statistically significant differences were found between patients with GLS and one-vascular, bi-vascular, or multi-vascular disease. In the control group (group 1), there was no statistically significant difference between patients with significant and non-significant coronary artery lesions in terms of regional longitudinal strain, regardless of the artery involved. We found that speckle-tracking echocardiography (STE) is a sensitive and less specific indicator of myocardial dysfunction that cannot distinguish between patients with 1-, 2-, or 3-vessel coronary artery disease and that it is a highly sensitive and less specific indicator of myocardial dysfunction. Speckle tacking Echocardiography (STE) is a sensitive indicator of myocardial dysfunction which includes ischemic coronary heart disease, and more useful for the detection and localization of proximal CAD lesions than the distal lesions. The global longitudinal strain is sensitive for detection of CAD and has a strong negative predictive value so it can be used as screening test and may rule out ischemia in patients provided with symptoms suggesting IHD.

Title : Numerical modeling of parameters and simulation to estimate insulin sensitivity and requirements in type 1 diabetics

Abstract :

Mathematical models make it possible to quantitatively simulate clinical situations and to test various hypotheses. The determination of insulin doses and sensitivity in type 1 diabetic patients is made on the basis of calculations after the fasting test and after estimation of the carbohydrate ratios. The objective of our study is to build a mathematical model describing the doses of basal and prandial insulin and the sensitivity to insulin, in order to propose a therapeutic scheme adapted to the needs of the diabetic patient and this through a mathematical model describing type 1 diabetes. The study focused on the files of diabetic patients followed up in the endocrinology department between 2015 - 2021, and who benefited from functional insulin therapy. The variables analyzed relate to patient age, sex, duration of diabetes, HbA1c, weight, BMI, total insulin dose, prandial insulin dose, basal insulin dose, the number of daily 10g serving, insulin sensitivity. A mathematical analysis was performed which allows to numerically estimate the dose of basal insulin, rapid insulin and K sensitivity for each new patient. The results of the analysis were then tested on an Excel spreadsheet, then used to build a web-based calculator application. We included 105 patients in the study. The set {V_1,V_2,⋯,V_105 } constitutes the table of the database considered in this study. Let P be a patient who is not from the database: P∈‍{P_i | i∈{1,⋯,105}}. Therefore, to determine basal insulin IB, rapid insulin IR and sensitivity Ksensitivity, we find the closest patient in the database according to the chosen factors. These patients are found by minimizing the cartesian distance between the factor vectors, these distances are defined as: d_1 (i)=√(〖(〖Age〗_i-Age)〗^2+〖(〖Weight〗_i-Weight)〗^2+〖(〖HbA1c〗_i-HbA1c)〗^2 ) for the Basal Insulin. d_2 (i)=√(〖(〖Age〗_i-Age)〗^2+〖(〖Weight〗_i-Weight)〗^2+〖(〖NS〗_i-NS)〗^2 ) for the Rapid Insulin Once we found the minimal distances and best values for Rapid and Basal Insulin dose, we can calculate the Total Insulin IT and deduce the K-Sensitivity defined as: K_sensibilité=I_T/I_Th These formulas were tested on an Excel spreadsheet, then used to build a Web-based Calculator application. The model obtained from this mathematical simulation will make it possible to reliably estimate the doses of basal and prandial insulin and the sensitivity in T1D patients.

Title : The Effects of Cardamom Seeds Extract (Amomum compactum) on Malondialdehyde, Histopathology and Testicle Weight: Experimental Study on Male Obesity Wistar Rats

Abstract :

In men, obesity is associated with low testosterone levels. In massively obese individuals, reduced spermatogenesis and associated with infertility. One infertility mechanism is due to the highly reactive oxygen species in obesity. A recent study showed that cardamom seeds could help improve health due to their anti-inflammatory and antioxidant properties. To analyze the effects of cardamom seeds (Amomum compactum) on malondialdehyde, histopathology, and testicle weight of Wistar rats induced obesity. This study was using a true experimental with a post-test design. A total of thirty Wistar rats were randomly divided into Standard group (KN), Obesity group (K-), and Treatment groups (obesity + cardamom 45 mg/kg BW/day) P1, (obesity + cardamom 90 mg/kg BW/day) P2, (obesity + cardamom 180 mg/kg BW/day) P3. Obesity-induced by given high fat and carbohydrates diet. In the end, the testicle and blood sample were taken for examination. Testicle samples were weighed and stained using Hematoxylin and Eosin stains. There were differences in histopathology Johnsen score, MDA level, and testicle weight between groups K- and P1; P2; P3 (p<0.05). Furthermore, MDA level and testicle weight between groups P1; P2; P3 (p<0.05) were different. Furthermore, there was no difference in Johnsen's histopathology score between groups P1; P2; P3. The higher dose of cardamom seeds gave better Johnsen score, MDA, and testis weight. Cardamom extract seeds effectively reduce testicle destruction by antioxidant and anti-inflammatory mechanisms due to improving Johnsen's histopathology score, testicle weight, and lowering MDA level in obese rats’ subject.
