Title : Clinical Profile of Children with Atopic Dermatitis in Basrah, Iraq

Abstract :

Atopic dermatitis is one of the commonest chronic inflammatory skin diseases. There are variable diagnostic criteria that have been developed to make the diagnosis accurate and thus, appropriate management would be established. Hanifin and Rajka diagnostic criteria was one of the most commonly used diagnostic tools. In this study, the researchers aimed to explore the clinical profile and the distribution of these criteria among children with atopic dermatitis in comparison with a control group. The study was conducted in the outpatient clinic of a Alfayhaa Teaching hospital in Basrah, Iraq during the period from December, 2020 to March, 2021. The study found that maternal education, positive family history of atopy and being an overweight or obese child were associated with atopic dermatitis. The study also revealed that xerosis, early age of onset, facial erythema or pallor, previous hand or foot dermatitis, and elevated serum Immunoglobulin E level were the most common minor diagnostic criteria. Further studies to compare Hanifin and Rajka diagnostic criteria with other sets of criteria is recommended.

Title : Ratio of Male to Female Infertility in Baghdad Al-Karkh (2015-2020)

Abstract :

Infertility is a common problem worldwide as male, female infertility or combined infertility which has psychological and social impact. Genetic, diabetes, pituitary factors, toxins, sexually transmitted disease (STD), idiopathic cause and many important roles can cause female infertility and male infertility. Problem diagnosis include, semen analysis, for male, female partner tested for hormonal para meters, and other parameter can help to define and treat infertility. This study included 1724 infertile female and male, was carried out in the High Institute of Infertility Diagnosis and Assisted Reproductive Technologies \ AL- Nahrain University, and Um Al-Baneen of infertility center / Baghdad-Iraq as these centers attract the most of infertile capsules in al Karkh side of Baghdad. The goals of this study is to know the ratios between male to female infertility in Baghdad, near Karkh, specifically for the years 2015 to 2020. Due to the large number of cases of infertility for both sexes recently. The results was found that the rate of infertility is similar between males and females in almost all years except for the year 2015. The female infertility rate prevailed over male sterility in a small percentage.

Title : Role of Doppler Ultrasound Surveillance in Early Detection of Vascular Complications among Liver Transplant Recipients

Abstract :

Living donor liver transplantation (LDLT) has been the last resort to treat endstage liver disease with improved success rates, provided the recent advances among the surgical technology, radiological appliances and new immune-suppressants. Vascular complications among liver transplant recipients are noted as a major threat to recipients’ survival. Slender anastomotic vessels make recipients more prone to complications through the reconstruction process. Routine postoperative Doppler ultrasound surveillance is believed to play a major role in early diagnosis of complications through intra-operative and early post-operative assessment allowing timed accurate management and increasing chances of survival for the liver graft and recipients. This prospective work is done on 45 cases with end-stage liver disease for whom adult LDLT is performed. Forty-five liver transplant cases were included in our study. Routine Doppler ultrasound surveillance diagnosed twelve patients to have vascular complications. Further evaluation of these cases by triphasic CT study or by surgical exploration confirmed eleven out of the twelve cases to suffer true vascular complications and only one false positive case was found free from complication. Thirty-three cases were marked by Doppler ultrasound as free from complications and one false negative case was found. Routine DUS surveillance of liver graft recipients have 91.67% Sensitivity and 96.97%Specificity to detect vascular complications. Intra-operative assessment and early postoperative surveillance of adult living donor liver transplant recipients using Doppler ultrasound can detect or exclude vascular complications with high accuracy.

Title : Infected Cosmetic Nasal Implant with Multi-Drug Resistant Organism: A Pandemic Delay

Abstract :

Nasal dorsal augmentation is popular among Asian population compare with Caucasian where they more towards reduction and corrective rhinoplasty. The implants can be autografts (derived from the patient’s own tissues); homografts (derived from tissues obtained from a different donor of the same species) and alloplasts (implants that are either semisynthetic or entirely synthetic). We present a case of infected cosmetic nasal implant in a 35-year-old female. She presented with nasal pain and discharge which started with nasal tip discomfort and redness almost 6 months with history of nasal surgery for one year. After she failed to response to multiple courses of antibiotics, a computed tomography revealed a nasal prosthesis at the nasal dorsum with possible sign of infection. Due to COVID-19 pandemic, there was delay seeing specialist for revision rhinoplasty. Later the nasal implant was removed under general anaesthesia by an otorhinolaryngologist considering the delay with the risk of ascending infection to cavernous sinus and osteomyelitis. The clinical features, imaging and urgency to be treated were reviewed.

Title : The Impact of a Clinical Pathway on Treatment Outcomes of Patients with Periodontitis in Public University Settings: A Quasi-Experimental Study

Abstract :

This study tested a newly-developed clinical pathway (CP) to manage periodontitis patients against patients managed using usual care. This study was a multi-center quasi-experimental involving newly-diagnosed chronic periodontitis patients. Five dental schools at Malaysian public universities participated in the study. A total of 36 patients participated as a test group and received non-surgical periodontal treatment based on the CP and reviewed at 6, 12 and 24 weeks. Treatment outcomes at each review were compared with data of 36 patients who had been treated without CP and matched as historical controls. Outcome variables were clinical parameters namely probing pocket depth (PPD), clinical attachment level (CAL), gingival bleeding index and visible plaque index. Improvements in these parameters for both groups were compared using independent t-test for normally distributed data and Mann–Whitney U test for non-normally distributed data. Both groups showed statistically significant post-treatment improvements for all clinical parameters. The CP group had significantly reduced number of sites for PPD ≥ 7mm and CAL gain compared to control group at 12th week (p=0.040) and 24th week (p=0.041), as well as between baseline to 24 weeks (p=0.039) with Cohen’s effect size 0.87, respectively. The use of a CP could lead to significant clinical improvement for patients with chronic periodontitis. Its use could be recommended in dental settings involving multiple operators to ensure best outcomes based on standardized optimal care and shared guidelines for clinical decision-making.
