Title : Liver Trauma Among Obese: A local retrospective Study

Abstract :

This retrospective observational study was conducted at a tertiary care hospital in Saudi Arabia in 3 years’ period from January 2017 to December 2020. The records of liver trauma patients who had a body mass index (BMI) >35 and having isolated blunt hepatic injuries were studied. Collected data included; their clinical condition upon arrival to the emergency department as well as the conducted investigations. Forty-six patients had liver injury. They were 38 males and 8 females with a ratio of male 6 to 1. Their mean and median age were 30.02 (range = 16 to 67) and 26 years, respectively. Their BMI ranged from 38 to 63. Eleven (23.9%) patients had comorbidities like hypertension, diabetes mellitus, asthma plus others. The most common mechanism of injury was road traffic accident in forty-two (91.3%) patients. Four (8.7%) patients suffered of direct injuries due to insults and quarrels. Twenty-eight (60.8%) had associated extra-abdominal injuries, while 18(39.1%) had no extra-abdominal injuries. Thirty-five (76.1%) had isolated liver injuries, while 11(23.8%) had associated injuries. They were Spleen trauma in 6 (13.1%), kidney in 2(4.3%) and retroperitoneal hematoma in 3(6.5%). Those with liver trauma were; twelve (26.1%) with grade I, fifteen (32.6%) had grade II, seven (15.2%) had grade III, ten (21.7%) had grade IV, two (4.3%) had grade V and none had grade VI liver injuries. Seventeen (37%) patients were managed by surgical intervention, while 29(63%) underwent a non-operative conservative management.

Title : Thoracomphalopagus conjoined twin: a case series

Abstract :

This study will describe some cases in thoracoomphalophagus conjoined twins. The description consists of patient characteristic, the factor that influenced mortality and the reconstruction method for chest wall defect closure. The patients were referred to hospital with thoracoomphalopagus conjoined twins. The variety of diagnostic procedure was used to measure the complexity of congenital abnormalities. Our team analyzed and decided whether the patients were operable or not. In conjoined twins’ separation procedure with large chest wall defect, we used the combination of mini plate and mesh for chest wall reconstruction. Among our conjoined twin’s patients, there was inoperable patients due to the complexity of heart defects. Among the operable patients, there was a patient with large chest wall defect that has been repaired with our original method. Sepsis was a major complication on post- operative care.

Title : The Resiliency of the veteran Spouses whose husbands suffer post-traumatic stress disorder: (A qualitative study)

Abstract :

Veterans’ spouses with post-traumatic stress disorder (PTSD) face many problems and stresses in their lives. They are often uncertain whether to leave or stay with their husbands. Therefore, this study aimed to explore veterans’ spouses' experiences living with their husbands who have PTSD. This qualitative study was conducted using the content analysis method. The participants consisted of 16 Spouses of veterans with PTSD. The data was gathered by semi-structured interviews and purposeful sampling and continued until data saturation. Data analysis was done simultaneously with the data collection. Main themes consisted of adaptability, spirituality, sympathy along with tolerance, and insufficient support, which together showed the concept of “Resiliency” in veterans’ Spouses. This concept leads to adaptability and continuity of marital life. Veterans’ Spouses experience many marital life problems and using defensive mechanisms and adaptability strategies play an essential role in their life promotion and satisfaction. Resiliency as the primary strategy leads to understanding the spouses’ needs and strengthens interpersonal relationships, intimacy, and life satisfaction.

Title : The effect of the spiritual care model based on the righteous heart on the post-traumatic stress of veterans in 2019

Abstract :

Post-traumatic stress disorder (PTSD) is an essential complication of war, making people who have experienced a war susceptible to secondary diseases. Since spirituality as an influential factor can help control stressful situations, this study was conducted to investigate the effect of spiritual care based on righteous heart on veterans' post-traumatic stress. This experimental study was conducted in 2019 on 60 veterans referred to Mashhad Army Hospital. Sampling was done randomly in two intervention and control groups. The intervention group received righteous heart-based spiritual care training in five 45-60 minutes long sessions over 5 weeks. Data collection tools included demographic questionnaire and post-traumatic stress scale. The collected data were entered in SPSS-20 software for analysis using independent t-test, paired t-test and ANCOVA test at the significance level of p<0.05. The mean score of post-traumatic stress before the intervention in the intervention and control groups was (52.30 ± 6.01) and (52.63 ± 4.75), respectively, and there was no statistically significant difference between the two groups in that regard (p = 0.813). However, after the intervention, the mean score of post-traumatic stress in the intervention and control groups was (42.27 ± 3.61) and (51.20 ± 6.18) respectively, and there was a statistically significant difference between the two groups in that regard (p <0.0001). Considering the results of this study, it is suggested to provide the necessary condition for veterans to participate in spiritual activities, which can improve their psychological status.

Title : THE ROLE OF TELE-MEDICINE FOR HOME-BASED PULMONARY REHABILITATION OF COPD PATIENTS: EXPERIENCE DURING COVID-19 PANDEMICS

Abstract :

Patients with chronic obstructive pulmonary disease and pulmonary rehabilitation programs require continue program for increase exercise tolerance. Quadriceps muscle dysfunction occurs in 30% COPD patients, regardless the severity of COPD. Pulmonary rehabilitation may reduce symptoms, increase exercise capacity, and improve the quality of life of COPD patients. During COVID-19 pandemics, home based management with tele-medicine approach are needed to prevent the transmission of the disease. This study aimed to analyze the effect of quadriceps strengthening and pursed lips breathing (PLB) as home based pulmonary rehabilitation on metabolic equivalent tasks (METs), exercise capacity, and geriatric depression scale modification (GDSM) in stable COPD patients using tele-medicine program. Of thirty four patients randomized pretest and posttest study was conducted in COPD patients at dr. Moewardi Hospital from July to September 2020 by using consecutive sampling technique. The study subjects (quadriceps exercise n = 17, PLB n = 17). The evaluation of METs and exercise capacity used Cahalin formula divided by 3.5 and 6MWT, respectively. Geriatric Depression Scale Modification was assessed at baseline and after completing the 6-week study. The first group had higher scores of METs (4.93±1.163 vs 3.94±0.431), longer distance of the 6MWT (507.94±53.358 vs 350.29±90.389 m), and lower score of GDSM (3.53±0.514 vs 4.29±0.920) compared to those of the second group (p<0.05). This study recommend the Quadriceps strengthening and PLB as pulmonary rehabilitation during pandemics of COVID -19 for stable COPD patients monitoring using tele-medicine.
